I have another format I came up with. I know its not practical but I think it may be a bit of fun to see in action and requires a bit more risk in order to win the big money.
Its called the Choose Your Destiny format (Working title, leave comments if you have a suggestion) and it works like this.
The usual 12 question levels are in place but there is a catch, for every question you get to see the category and get to pick the difficulty of the question:
Easy (Questions 1-5 only): These are like the starting 5 questions from the 15 question format. The cash value is low but can be used as a easy way up the cash ladder, however this path is locked out after question 5.
Medium: This is basic Millionaire questions and average cash value.
Expert: These are like the higher questions on the basic Millionaire cash tree (Q11-15) but have a higher cash value.
Super: The highest difficulty and are like questions from The Next Dimension (Super Millionaire Q7-15) and have a huge cash value boost.

Every question you get right, the cash allocated to it goes into your bank which you can redeem at the safe havens.
Lifelines
Starting: Ask The Audience, 50/50, Phone a Friend
After question 2: Double Dip, Level Switch
After question 7: Revival, Switch the Question
Level Switch: After seeing a question, you have the choice to switch the difficulty down one level as well as the cash belonging to the level. This is used as a panic button in case if you have absolutely no clue about the question.
Revival: Once you get to level 7, you have the chance to revive one USED lifeline for another use. If no lifelines have been used, $50,000 will be added to the bank.
