I came across this old Millionaire Australia promo showing a phone a friend who missed the call.
Has this ever happened any other times in the shows history?

It would be a bad break for the contestant, that’s for sure. Apparently the lifeline is considered to be used even in the event of not getting a response. This was perhaps most famously enforced in a sitcom dating back to US Millionaire’s early days (see the video below):

The only fair solution that I can see in the actual show is that the producers would tell you to try to call one of the other Phone-A-Friends that you have. (You have three to pick from, later on like five.) Like if the person at the switchboard cannot contact a PAF selection, they'll relay back to the director who will likely attempt to redo that moment.

Now if some jackwagon PaF hung up in the middle of the thirty seconds like Fox did, not sure.

This has happened a few times in the Finnish version. On 29 April, 2000, a PaF friend of Jarmo Jalonen didn't answer the phone, so he called another one instead. There's a very short clip of the incident right in the beginning of this video: youtu.be/zKCyxWbEUzYOn 5 November, 2016, a PaF friend of Katariina Keinonen didn't answer the phone (his phone couldn't be reached), so they called him until he finally picked up. On 18 November, 2017, a PaF friend of Mikael Nyberg didn't answer the phone (his phone couldn't be reached), so they called him until he finally picked up. The friend was in a ship at the moment. He gave Nyberg a wrong answer, and Nyberg walked away with nothing.
