Alright, this happened a while ago (I mean nearly a year ago, I think) but it just popped into my head out of nowhere. I was watching Millionaire, and they went to commercial. All was normal, nothing out of the ordinary. And then, for about half a second, the words "TICKET PLUG HERE" flashed on a black screen. There was barely enough time to read them before they flashed away and the first commercial came up. I thought it was a pretty funny goof.
Has anyone else seen this happen? Does it happen often, or was it more of a one-time thing?

I remember that happening on one of the season 8 episodes. I guess whoever was editing the episode made a mistake when attaching the ticket plug to the actual episode. It happened so fast that if you blinked, you probably missed it, but I remember spotting it and chuckling to myself.

A ticket plug is where Meredith comes on and says her "Be in our studio audience, come get tickets" message.
Someone along the edit chain failed there I guess.
